Please see below list of requirements:

 

1.   Duly Accomplished Form

2.   Two (2) passport-size or 2x2 colored photo with white background

3.   Airline E-ticket or itinerary

4.   Hotel confirmation or Tour voucher

5.   Actual Valid Passport

6.   Photocopy of the front page and emergency contact information page in the applicant's passport

 

 

For New Applicants the following are the additional requirements.

 

7.    Recent ITR (2011)

8.    Certificate of Employment

9.    Photocopy of company ID

10.  Bank Certiicate with OR and with the following info: type of account, opening date, average daily balance (should be at            least 100,000) and current balance

11.  Original 6 Months Bank Statement from the bank (detailing all transactions)

12.  Original Passbook with detailing transactions for 6 months (original and photocopy of all transactions)
